Brilliant surgeon Goto Kensuke receives an invitation to manage a remote clinic on Yonaguni Island in Okinawa as a result of a serious mistake he made in Tokyo. However, this comes with a challenge of finding medical staff that can be relied upon in such an isolated location, causing locals to undertake long boat trips to access medical attention. Despite grappling with sea-sickness and being unlicensed to drive, Goto proves his expertise and resolve, winning over the skeptics among the island's inhabitants. Although the children mistakenly refer to his facility as "Koto Clinic" and even create a banner for it, Goto remains committed to his noble calling on the island.
