Known as the "Slit-Mouth Woman," Kuchisake-onna declares herself a living urban legend that stems from people's fears and superstitions. Her reputation as a horrifying figure is diminishing as her legitimacy is being questioned by the public. To make matters worse, a 13-year-old boy named Kouichi Sano not only dismisses her mouth deformity but also teases her with the endearing nickname "Miroku-chan." As a result, Kuchisake-onna ponders whether she is becoming an inadequate monster or if Kouichi sees her as a true ally.
